We conduct a comprehensive client assessment to ensure you receive the best care tailored to your needs. One of our professional Registered Nurses (RNs) will visit you to assess your situation and develop a personalized care plan.

During the assessment, we will evaluate the following:

Client Information – Name, date of birth, contact details, and relevant medical history.
Activities of Daily Living (ADLs) Assessment – Assistance needed with personal care tasks such as bathing, dressing, grooming, eating, and mobility.
Instrumental Activities of Daily Living (IADLs) Assessment – Support with daily tasks like meal preparation, housekeeping, medication management, and transportation.
Home Safety Evaluation – Identifying potential hazards and recommending modifications to enhance safety and accessibility.
Care Plan Recommendations – Developing a customized plan to meet your unique needs and improve quality of life.

Personalized Care Plan

A Personalized Care Plan is a structured, customized roadmap designed to address a home health client’s unique needs and preferences. Based on the Registered Nurse's assessment, this plan outlines the type and level of care required to enhance the client’s well-being, safety, and independence.

Tailored Support – Every client has different health conditions, mobility levels, and daily living needs. A care plan ensures they receive the right assistance with tasks like bathing, dressing, medication reminders, and meal preparation.

Enhanced Safety – By assessing the client’s home environment, the plan identifies potential hazards and includes recommendations to prevent falls or accidents.

Improved Quality of Life – Whether a client needs companionship, help with housekeeping, or personal care, a structured plan helps maintain their dignity and independence while receiving compassionate support.

We employ Certified Nursing Assistants (CNAs), Personal Care Aides (PCAs), and other Companion Aides, all of whom are professionals experienced in providing compassionate, high-quality care. 5. Will my caregiver be assigned to me long-term?

Yes, we are a participating provider with Virginia Medicaid and accept most Medicaid Managed Care Organizations (MCOs). We also offer private pay options, including credit cards, debit cards, Apple Pay, and ACH bank payments for added convenience.

8. How are your services billed?
Services are billed based on the Personalized Care Plan developed during the Home Care Assessment. We offer flexible payment options and send invoices directly to your mobile phone or email for secure and easy payment.

9. Can I adjust the care plan if my needs change?
Yes, we regularly reassess your care plan to ensure it meets your evolving needs. If your situation changes, we’ll update your services accordingly to provide the highest level of care.
